I saw that too, if we're thinking about the same incident. Poor woman was hit by a flying CO2 bottle that came unscrewed from its valve. From the looks of it, the fool who installed the anti-siphon tube didn't use locktite.

Ist point: Presets rarely if ever give the stated output, a great many are hugely different from what they are declared to be...that plain enough.
2nd point: See point 1
3rd Point: You should not use loctite on a valve, torqued to spec but no loctite and even a torqued valve will over time loosen and the nature of screwing a preset on and off the ASA regardless of an on/off tap being in place will accelerate this ...that was the reason for raising the issue...and if you do you run the risk of the bottle failing a hydro retest...CO2 bottle require this too
