i have one and my teamates have used them for years.
Their is no proof unfortunately as to what was the problem with the other bottles or if it was even the bottles and not the regs.

from the manual i see that the "working pressure" 30 MPa which equals about 4350 PSI.
but are designed to work even at pressures of 45 MPa = 6525 PSI
and somewhere i read an artical by Hpac saying that a stako withstood (sp) 9000+ psi

Last UKM I attended, man that guards the fill rig told me 4.5k is fine.
I then proceeded to fill my Stako to 4.5k, and lived.
Edit: This was in April 08, so unless the rules have changed since then.. that or Hpac are wrong.
