2 Why you smoke
3 How long you have smoked
4 Have you ever tried to give up
5 How many packs you smoke a week
6 How much you spend a week on cigarettes

I hate school work but it has to be done, otherwise mum said i cant play 'ball anymore!! Need to get good grades!
