Which part is 'loose'?
Is it just that the reg (the black front grip) wiggles slightly when screwed all the way in to the ASA (the red part that it screws into)?? If so, you needn't worry, as once it is aired up it will stay in place.
If it is that when you screw the reg into the ASA, the whole thing (reg and ASA) continues to spin, then it means that the screw that holds the ASA to the body is loose, you just need to remove the reg and tighten up the screw inside.
There is a section of the front reg which is designed to spin freely, this is the section where the elbow is attached. This is so that you can position the elbow wherever you like.

As for the throttle system, it is most odd that it should stop filling. It sounds like the fill nipple has jammed. Have the store you purchased it from call me, as they may not have a replacement. If this is the case, then I will endeavor to get you a working throttle system ASAP. What size tank is it that you have, 45ci or 68 ci?
With regards to efficiency, this has many variables. Most site in England will only fill to 3k, so a 68ci tank @ 3000psi, on a box fresh PMR, 500 shots sounds about right. This will improve with time.
