Get the chaos board, a set of eyes(you can use timmys eyes or buy them from digikey) and get someone to make the holes.

sorry philip, my question was not clear, i meant where do you purchase the board and eyes from? how big are the holes? i am an engineer by trade, so doing the work is not a problem.
