they do sometimes just go - it is a little copper disk that flexes with the changes in pressure - rupturing if it is too high. however the flexing may produce stresses that will weaken the disk if the pressure changes are done too quickly - ie: fast fills or empties. so could just be one of those wear and tear issues. just be sure you fill slowly and don't vent the bottle at high speed.
