screen printing onto a jersey (query)

i have a 06 distortion eclipse jersey in yellow and i want

'DAVE CLARKE'
'EvoOnline'

printed on the back of it. anyone know of a company in the UK with reasonable prices and who perhaps you've dealt with in the past for such stuff. if you do know of anyone then also do they print logos and the such.
