re-hydro on tank

hey guys sorry to be a pain but i need to know when the re-hydro for my tank will be, its an SCI 1.1 ltr lots of numbers include:
UNF625 USA SCI ALT756E-3491 FIN 2019/04
PW310 PH465 BAR 1.0 KG V1.1L AA6061
EN 12245 0044-411/03D 2004/05 07

so i take it i was born in may 2004? and will die april 2019? any ideas if the hydro is 3 or 5 years etc? sorry its all rubbish 2 me! cheers
