I heard a rumour there has been a further Stako failure. This time a complete rupture of the tank.

Is this issue that we don't know about yet do with all regs or just regs fitted to stako tanks. I was reading somewhere about the fact that a reg is not subject to being tested, yet it holds back 4500psi so it should be tested?
Am i close?
